My fiance had her interview yesterday and she was approved.
After the I-129 paperwork made it to the Embassy, we had a few challenges because there were quite a few details left out of even the best guides. FYI.. Madrid will only communicate by Email, but they are good about responding quickly.
The Medical exam was the most difficult part to figure out. In Madrid, there is only one doctors office that can do the exam and the exam needs to done at least a week before your interview. My fiance had to fly to Madrid for the day and have the exam, then we picked up the medical packet on the day before the interview (about a week later. So expect to make two trips or if your timing is good you may be able to just stay in Madrid for a week, getting the exam one day and the interview a week later, but the Medical office will be working on Spanish time so you are not guaranteed when the papers will be done.

I had my appointment at 11:30. I got there 11:15 and they let me in, went through security, took my phone, etc.. I was a little nervous and I forgot my jacket that I got later when a woman came into the room asking for its owner.. haha. When I was inside everything went very smoothly. After 15 minutes I was called to give the proof of payment, my medical, passport, etc, to a Spanish very nice lady. She explained me that when I got the visa it'd be stappled to a brown envelope that I wouldn't be able to open because I'd have to give it to the officer at the POE in the US. Then she told me to sit again and wait until the consular office called me. After 20/25 minutes the consular officer called me to the window and took my oath and fingerprints. Then he started asking me in Spanish, but I felt more comfortable speaking English, so I started speaking in English. He continued in English after that. My appointment was at 12:30. I arrived at 12:15 and they let me in. I went through security and they kept my phone. A lady at the lobby asked for my name and passport and they gave me a turn number. I went in the waiting room and it was almost empty. They called me at 12:30 to the first window where a lady took my passport, the visa payment confirmation and my medical exam documents. She gave me back my original birth certificate that I sent with packet 3, the sealed xray cd and an IMBRA pamphlet. She then told me to sit back and wait to be called again. After only 5 minutes they called my number and I went to a new window with a very nice young lady. She asked me if the interview could be in English and I said no problem. She took my oath and scanned my fingerprints. Overall the staff were friendly and professional. My interview was really long and they kept asking me about everything since we met till our plans for marriage. I had to send one more affidavit of support because mine didn't meet the requirements. But received the visa and now we are married in the US.

My mother had her interview in Madrid today and the experience was excellent. We arrived at the Embassy 25 minutes early and were let in immediately. I showed my US passport and was let in with my mother without any questions. The Spanish security could not have been more helpful and we were only asked to leave our cell phones with them. We selected a number for immigrant visas on entering the room and found there were only two others for our category in front of us. We waited about 25 minutes for our number to be called and I was able to accompany my mother to the counter. The lady couldn't have been nicer as she requested and checked off each of our original documents with those forwarded to them by the NVC. She returned the originals to us except for the passport and asked us to take a seat again. This took about 10 minutes.
