We had our application expedited at NVC for a medical issue, and because the applicant's job was ending soon, causing financial difficulty for us. The expedite was never mentioned at the interview.
1. Security check
2. Check in with person at the front, show the appt time confirmation email
3. They give you a folder to put your documents in a certain order
4. You will be called almost immediately, you give your papers at the window and then sit and wait
5. After about 2 hours, they call again and fingerprints are taken
6. Wait another 30 minutes
7. Interview
Interview is really easy. Only the applicant can be at the window.

Where did you meet him?
Were you studying in X at the time?
Does he still live in X?

Very smooth experience. Security and embassy staff were all very polite and professional.
The appointment was at 9AM. We arrived a few minutes early and were allowed to enter. There was no line to get in. Security was much more forgiving than specified on the embassy web site. We were both allowed to enter. We didn't bring any bags but other people had regular backpacks and such.
Waiting time was approximately 90 minutes total with three trips to the window: 1) submit documents, 2) verify identity (fingerprinting), 3) interview
The interview was very straightforward and took only 3-5 minutes. The interviewer was not Japanese but allowed the interview to be done in Japanese and had sufficient proficiency to do that. There were 10 questions, all routine (when/where did you meet family, have you ever been to the US, etc), mostly with yes/no or very short answers. The embassy was not busy in the morning hours on this particular day, so it was a 45 minutes or less total time inside.
First, paperwork. Second, Fingerprinting. Third, Interview. All of which didn't take long. The interview itself was about 5 minutes and my fiancee was nervous about it in the past week. With some encouragement and with some preparation, its an easy process. I checked reviews to try to get some insight on the interview itself, and I've noticed that there are questions that MUST be asked, so for anyone wondering what those are.

When did you first meet in person? (Question is likely dependent on if you met online somehow)
When was your last physical meeting?
Is this your first marriage? What about him?
Have you met his family?
Have you ever lived in another country?
What are his hobbies?

Total waiting time at the embassy was close to 2 hours. Interview itself took only 5min. They asked a few question about my husband and myself. It was easy and the officer was nice.
