5 stars for the interview at the Consulate
2 stars for the communication with the Consulate before the interview
1 star for the medical exam at Istituto Varelli
NVC sent us an appointment for an August interview only 3 weeks in advance. They scheduled the interview few days after both NVC and the Consulate in Naples told us that we would most likely get an appointment in September because the Consulate is closed during the 2nd and 3rd weeks of August for a vacation and almost all time slots for the last week of August had already been assigned. The appointment process is quite obtuse and you would be better off not trying to augur the interview date. Try not to make any travel plans for the months for which the interview might be scheduled. We learned this lesson the hard way.

After drawing on this wonderful community for so long, searching for and gathering so much information through your threads and explanations, I have decided to register just to offer a review based on my personal experience.
Me and my husband (he is the U.S. Citizen and Petitioner, I am the Beneficiary) returned from Naples, Italy, yesterday after being granted a CR-1 Visa!
First of all, friends, I would like to reassure you all: everyone, both at the Istituto Varelli and at the Consulate, was extremely friendly and I was made to feel at ease at all times. There is absolutely no need to worry yourself to death (however comprehensible it might be), since the entire staff involved in the process seems to be willing to help in every circumstance.

We just returned from our medical visit and successful interview in Naples and I thought our experience might be relevant or helpful for others in our situation.
I'm American, my husband is Italian, we have one child who has dual citizenship.
We've been together over 14 years and married for almost seven. I've lived in southern Italy for almost 8 years.
I read other reviews on this site about how long people waited and I was very nervous about entertaining and containing our 3-year-old for all of that time. We arrived at the medical visit by 7:30am, he took a number, showed his passport and we went to a waiting area (this took a whole 2 minutes). From that point, they didn't take people in the order they had arrived, which upset the people who'd arrived at 7 that morning. Today was the Visa interview culminating a 91-day preparation process. Since it went so quickly for us, we were worried a hiccup would occur resulting in the visa being held, but all went as hoped and the visa was approved.
We prepared thoroughly and reviewed the document requirements often to ensure we didn't miss any. My recommendation for document preparation is http://italy.usembassy.gov/visa/iv/application.html. I also recommend using the DoS Reciprocity Tables at http://travel.state.gov/content/visas/english/fees/reciprocity-by-country.html to ensure you request the Italian equivalent document - for example the correct Police Certificates.

I arrived at the Istituto Varelli at 7:50 and I already had people in line before me. They will take your passport and tell you where to wait, then a nurse will start calling people in the same order as they arrived and will take a blood sample. After that they'll take chest x-rays, check your vaccine records, take your payment for the examination and you will have you to wait for the actual examination -they just ask for a few things, check your weight, height and whatnot. I was out by around 12:30 and was told to go to the US consulate at 2:00 PM for fingerprints -this won't take long.
The day of the interview we were told to be at the US consulate at 7:30, although they won't let you in until 8:00. First they will call people to collect all the documents listed on packet 3: BE ORGANIZED AND KNOW WHERE THEY ARE.
