First, you can reach the "Istituto Varelli" from M5 Montesanto "Circumflegrea" 2 stops Saccavo and walk for 10 minutes from there. They say to come at 8.15 but I was there at 7.40 and number 12. They have a special line for the Consulate, take the number when you enter, but the line is not extremely strict, depending on what you have to do and how they handle the passports.
The exams are easy: blood test for syphilid, rx chest, a meeting with a clerk to print 3 papers for the doctor to fill up during the visit: 4 regular italian photos are required and a fee of 170 plus 60 if you need the two immunization: in that case they will provide a certificate for the vaccinations that is valid in US (a plus maybe for some jobs). Will ask about your address in Italy and US, so be sure to have it written. Finally the visit with the doctor, with height, weight, temperature, eyes, previous health problems, auscultation and at the end the vaccinations if you decide to do with them. June the 8th i had my medical check at Istituto Varelli. I guess all applicants had appointment at 8:00. I was there at 7:30 and some applicants were already there. They already have the blood sample taken. From 7:30 to 8:30 nobody was called for the blood draw. Then they called people according the order of arrival... more or less. Anyway I stayed there till 12:30 to have all the checks. At the end the MD didn't want to watch my filled DS-3026. He compiled in front of me something similar. I suggest to get and take with you any kind of medical evidence to confirm your health condition. Remember you need 4 51 x 51 mm pictures.
June the 9th I went to the Consulate at 8:00. In the Packet 4 email the appointment was at 8:00. Instead at the on-line registration for the courier the appointment was at 9:00. My experience was pretty smooth. Everything happened as expected and according to instruction received. Beside the required documents I recommend everybody to have some extra cash (the indicated cost of the physical was 160, while it was actually 170 -without additional vaccinations) and, most of all: bring a lot of passport pictures (5cm x 5cm), I mean: A LOT.
In the instructions received (and also somewhere in this blog) I read that 2 pics are required for the physical and 2 for the consulate.
The USCIS already asked me to send 2 right after the I130 approval. The day of the physical the guy there wanted 4 pics. The day of the interview they wanted other 2 pics. I was glad I had them because after all the paperwork it would be silly to fail the application for such a small detail.

We needed to extend our petition twice, and postpone our interview date due to a medical issue. However, Naples was extremely responsive and helpful through email. They only needed medical documents to extend the validity of the petition. Nothing unusual for the medical exam, the same as everyone's posts here. For the interview, he was the first one there and arrived about 1 hour and 15 minutes before the scheduled time. Unfortunately, I the K-1 petitioner was not able to accompany him and I think this made a difference. He had all the documents and way more than was asked for, but the officer asked him if he wanted to speak in English or Italian and he said Italian. The officer then said, how do you speak to your fiancé and he said in English but I can't speak very well. The officer then said ok so you can speak in English for your interview and he said ok but if I don't understand, then you can translate? and the officer said sure.
