We needed to extend our petition twice, and postpone our interview date due to a medical issue. However, Naples was extremely responsive and helpful through email. They only needed medical documents to extend the validity of the petition. Nothing unusual for the medical exam, the same as everyone's posts here. For the interview, he was the first one there and arrived about 1 hour and 15 minutes before the scheduled time. Unfortunately, I the K-1 petitioner was not able to accompany him and I think this made a difference. He had all the documents and way more than was asked for, but the officer asked him if he wanted to speak in English or Italian and he said Italian. The officer then said, how do you speak to your fiancé and he said in English but I can't speak very well. The officer then said ok so you can speak in English for your interview and he said ok but if I don't understand, then you can translate? and the officer said sure. SO he questioned him for thirty minutes in English about our relationship. He asked questions about my job, my brother, my mother, my uncles, and everything. He asked the exact dates I went to visit, even if my fiancé said she came to visit me in November 2013, he said what are the exact dates. My fiancé then mentioned that we speak in vibe and Facebook and Skype. However I had only included messages from Facebook and Skype because we didn't speak a lot in Viber, only in the last two months. He also asked to see my finance's old passport that had expired for no reason. He had it in the interview, but the officer wanted a copy of it. He also wanted the promise soggiorno from the time my fiancé was in Italy, since he is Moroccan. My fiancé gave him the 2014 permesso but he asked in English if he had the Italian residence permit from 2011 - 2014, and my fiancé said yes because he did not understand the "residence permit" to be the permesso soggiorno. He thought it was his place of residence. So he said yes, I have it even though he did not have this because when you renew your permesso, the police take your old one. So he was given the 221g for a copy of his old passport, his permesso from 2011 -2014, and Viber messages from 2011-2014. I sent an email that day to the consulate with our Viber messages and phone records and more Skype calls. I also sent a photocopy of the passport and explained that he did not have any permesso soggiorno from 2011-2014, only his current one.
Within 2 business days, the visa was approved.
Overall, emailing and contacting the U.S. Consulate in Naples was extremely helpful and responded to emails within one hour to 1 business day. However, the actual interview was not a great experience and could have something to do with the fact that I couldn't be there. Even though I wrote a current letter dated within the week that I was free to marry and still wanted to marry him and we both signed and dated it. It seemed the consular officer was looking for an excuse to deny it, but in the end it was approved very quickly and did not spend more than 2 days in administrative processing.

For anyone experience this, please know that if you have a real relationship and can prove this then there is no reason you will not get the visa. Just be calm and explain your situation and give them the documents they require. Not sure if the officer was just having a bad morning, but we knew there was no way they could deny us!
