The process takes two days. Day 1 is the medical and day 2 is when the actual consular interview takes place.

For the medical examination, I cannot stress enough how important it is to arrive early. On paper, you’re supposed to get there by 8am but I highly recommend you arrive way earlier than that. I got to Istituto Varelli at 6:30am and was the first in line. By 7am, at least twenty people were in line behind me. They began the whole process at 7:10am and being the first in line, I got done by 8:40am. Everyone was really friendly, precise and informative.

The day after, I showed up to the consulate at 8:15am (my appointment was at 8:45am). I was only let in 15-20 minutes before my appointment. After leaving our phones at the security checks, my fiancé and I got in. The 1st day the examination went well.
I recommend that you arrive early like 6.30 if you want to be done early, it is very crowdy.
also, it is better to have cash, otherwise if you want to pay with the credit card, you would have to go back to the front desk to pay, and people will get mad at you for skipping the line. If you have cash, you can pay where there prepare your file.
The interview:
you do not need to arrive early, just arrives 15 min before your interview time.
once you enter you will be called to check your papers, and that you have all you need.
then you will sit down till they call you again. this time they will take your documents as well your passport. they will take your fingerprints and ask few questions. then you will be asked to wait till they call your number again.
when they call your number you will be interviewed by the counselor. you will swear that you will say the truth. then the interview will start. I... read complete review

So, I thought I'd give my review too, in case it might help.

DAY 1 - MEDICAL APPOINTMENT at ISTITUTO DIAGNOSTICO VARELLI
Thanks to other reviews, I read to arrive early and so I did. I was there at 7.34 and I was n. 13. I took the taxi to arrive, which was a good idea. I was told buses might take a long time and you just end up waiting forever.
Anyway, when you go inside,this is what I did:
1. Select the ticket with the C option (consulate). They will call your number and make you sign a form and give them your passport (they will keep it and return it later).
2. Blood + Urine test. After waiting for my turn, a nice woman took my blood and she explained it was just to check I didn't have syphilis and gonorrea. I highly suggest you don't eat or pee before this part. (but bring some snacks for later! - I didn't and I was starving!)

MEDICAL VISIT @Istituto Diagnostico Varelli

The Varelli medical clinic is located quite far from the city center, although it doesn’t take long to reach by car. The instructions of the Consulate suggest you to get there by 8AM.
I arrived there at 7AM and I already had 5 people in front of me, already waiting. I highly recommend to get there early as well, as you will have to re-do the same line each time for each part of the visit (blood test, urine test, X-rays, vaccine examination, general visit)
I took a ticket number from the C (Consulate) button of the machine and waited in line.
A woman at front desk called my number and took my passport. They will keep your passport until the end of the whole procedure.
I signed a document and they sent me to a new waiting room, where shortly after I was called for blood and urine test. They need these tests to check you don’t have gonorrhea and syphilis.

The usual two-day process at the Naples Consulate for immigrant visas (my mom is a 10-minute walk from the US Consulate in Milan, but immigrant visas for Italy are processed exclusively at the Naples post).
She got there on Sunday late afternoon by high-speed train, checked in at a hotel 5 minutes away (on foot) from the Consulate.
Monday was the medical. Instructions say to be there by 8, however I told her to be there earlier because it's first come-first served. She was there at 6:45, clinic (Istituto Varelli) opened at 7, she was first in line. They checked the Consulate list (they schedule the medical for you for the day before the interview), she was on.
Blood and urine test right away, then chest x-rays.
