So I read up on this topic http://www.visajourney.com/forums/topic/551080-immunizations-ds-3025-or-i-693/ but I'm still a bit worried. Going down the line of vaccinations, there's only letters to the right under the blanket waivers column. There's no dates on my husbands immunizations, just the Letters. My husband said there's a vaccination he didn't have, but the physician told him that he is waived from having it. Will this cause any issues for us during AOS? Also, at the bottom on the right side it does state that it is requesting individual waiver based on religious or moral convictions.

That;s what I'm confused about. This is the DS-3025 that the medical examiner before his interview filled out. The immigration related doctor. I'm not sure about the waiver. The doctor checked that box (religious or moral reasons), and told my husband he was missing a vaccination but it isn't required nor do they make it available in that country. It's Saudi so I am assuming they didn't make that vaccination available because of religious reasons?? Also, there's no dates either for any of the vaccinations. He only filled out the box to the right.

It is bad that the doctor did not put the dates he gave him the shots. If they were shots he had before, then good. He should still have the records for them. He needs to take his records to a civil surgeon. His Td is marked D, and so is the MMR. He will need those, and a flu shot.

He can get them elsewhere, like the health dept. to get them cheaper, and take the records to the civil surgeon to transcribe onto the I-693.

No idea why it is marked incomplete and requesting waiver for religious/moral reasons. The DS-3025 has to be filled out properly, and all required shots given and marked off correctly on it. The doctor filled out his form wrong, and so you must now find a civil surgeon that won't insist on a whole new medical and overcharging you, and get the missing vaccinations. Call around to find the best price. Sorry, I know it is a hassle.

Thank you so much for your reply. That's exactly what I was afraid of. He had the immunizations when he was a child, and why the doctor did not fill it out properly or fix the dates is beyond me. It's frustrating, but what can you do. I'll call around and hopefully we can find someone who is reasonable. Again thank you for replying.

We're in a similar situation in that Dee didn't get one of the vaccinations because it was not available in Indonesia. I'm hoping we can just get the vaccination done here ... I would hate to have the AOS declined because of one vaccination and have to pay that 1000 USD or so over again!

Yes, you'll need to have the vaccination done here in the US. That's what we had to do. Then find a civil surgeon that will do just the vaccination requirements, and not charge you for another full medical.

Did you already received an RFE for it? If not you can always chose to wait until they sent you one. The dates missing is pretty sloppy, but the "missing" shots are covered by the waiver request. They might just grand you the waiver. Also if you wait until the RFE, or until asked for during the interview, the flu shot might not even be required. Of course putting a price on "piece of mind" is hard, so if you are worried, get the form redone.

Hi Meg!Congratulations!I am currently sailing on the same boat. I am also wondering about this vaccination record. I guess your fiance and i had medical at the same hospital in Saudi Arabia. And had the same issue with MMR and Td. What did you do to resolve the issue?Did your fiance had to receive the vaccination here and see and civil surgeon?
