Had medical and don't understand what to do with documents

My fiancé had his medical last week for a K-1 visa. He did not take any vaccination records (he doesn't have them) and when he asked the doctor to give him the required vaccinations for AOS, the doctor said he doesn't do them at the clinic and told my fiancé to go get his vaccination records and take them to America for when he does AOS. At the end of the appointment the doctor gave him a ds-3025 which was blank other than the doctors signature and stamp and also gave him a computer disk.

What does he need to do with these? He is leaving Friday to come to the US and we are trying to make sure he'll have everything he needs for AOS

If he brings a vaccination history to America will the Civil Surgeon just write that info onto the necessary documents? Or will he need to redo the vaccinations in America?

If he has all the neccessary vacinations records the Civil Surgeon will just fill out the forms. No need to do them all over again.

Your Fiance needed to get his vaccination record from the physician at the place of his residency BEFORE the medical examination. And during the exam the record would been simply transferred to your Fiance DS-3025.

So, yes, he should definitely get the record before he leaves Ukraine. Just make sure his physician uses "Latin", not just Ukrainian - that the US civil surgeon could identify the vaccinations your Fiancee got.
