I-485 Medical Exam Vaccine Supplement

Hey all! Finally got the fiance here on the K1 visa -- we are getting married Saturday and have begun filling out the forms. The one SNAFU I've encountered is his medical exam. On page 4 it states he is not required to submit an entire medical report as he has had his medical examination just recently for the K1 Visa. However, it's still asking for the Vaccination supplement. How on earth do I get this? We are thinking his medical exam report was in the sealed packet he submitted when he went through Port of Entry immigration, and they didn't give anything back to him - so we have nothing in print proving he got the vaccinations, though the sealed paperwork they took from him says he did. Recommendations? It'll probably be a couple weeks before we actually submit since we need the certified marriage license yet.

Hi Krschweit!

At the end of my medical exam, the doctor gave me a form entitled "Vaccination Documentation Worksheet" (on it is is indeed written: give copy to applicant). There is my photo on top of the document and it has 2 pages. This is what I sent (at least..a copy!) with my application after getting married. I sent it more than a month ago and still have not received a RFE so I guess it was this document. I would therefore advise your fiancé to contact the doctor's office that he visited for his medical check-up, so they can send him a copy?

I was given a copy of mine by the embassy and the doctor who performed my medical, perhaps get in contact with the doctors office and ask for them to email or email/fax a copy to you.

It is the DS3025 that the medical facility should've given your fiance at his medical exam. I made sure my fiance got his to bring with him. You do not get anythng back from the packet that was given at POE.
