I've just gotten married under a K-1 visa and am applying for a change of status to permanent resident.

I have my DS-3025 (which shows all vaccinations completed) and it was completed only a few months ago (so it's still valid).

1) Do I need to submit an 1-693? (half of the registered doctors in my area refuse to do it without a full medical which I don't need and the rest just sound confused).

2) If I do need to submit it, exactly what parts need to be filled out? Do they just need to transcribe the info on my vaccines etc. or leave it blank?

I feel bad to ask this question when I know other people already have but I've looked through pretty much every forum but I really haven't found a clear answer for my situation (all vaccinations given, DS-3025 available etc).

If you are a K-1 fiancé(e) or K-2
dependent who had a medical examination within the
past year as required for the nonimmigrant fiancé(e)
visa, you only need to submit a vaccination supplement,
not the entire medical report. You may include the

vaccination supplement with your Form I-485

Yet visajourney's checklist states:

. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US.

Yes, you still need to submit the I-693. You need to submit parts 1 (personal info), 4 (civil surgeon info) & 7 (vaccine info). The civil surgeon will transcribe the vaccination info onto Part 7. We called around and found a civil surgeon who did the transcription section and charged $25.00.

No, you do not need to see a civil surgeon if you had all the required vaccinations marked off on a DS-3025 at your K-1 medical.

We just filed on Monday and only included a copy of the DS-3025.

2015-03-07: Got engaged (L)

2015-03-30: Sent I-129F to Dallas lockbox

2015-04-14: NOA1 Packet received, sent to CSC

2015-05-07: NOA2 Approved!

2015-06-15: Received Packet 3 by mail

2015-06-17: Sent Packet 3 to Consulate

2015-07-14: Received Packet 4

2015-08-10: Medical in Toronto

2015-08-16: Received medical envelope

2015-08-24: Montreal interview- APPROVED!

2015-08-27: Visa arrives at Loomis location

2015-08-28: Visa in hand!

2015-09-17: Entered USA

2015-10-23: Married!

2016-01-11: Sent AOS Package I-485, I-765, I-131

2016-01-21: NOA1 Package received

2016-02-19: Biometrics

2016-04-01: AOS interview- APPROVED!

Many people who have submitted there AOS have done the same and their timelines show no RFE so I'm thinking DS-3025 is fine especially since I had all my vaccines didn't need any administered.

Hold onto your hat its going to be a bumpy ride!!

For me I could never find the a "civil surgeon" that would be willing to transcribe the vaccinations on the the supplement. They say...oh we have to see you and do the actual physical. I wasn't wanting to pay the extra fee. So we handed in only our DS-3025 that had all the required vaccinations marked off and handed it in without the I-693. We were approved 4 months later without interview and no RFE. We followed the links provided above and were successful. I think you will be fine without the I-693 as long as all the required vaccinations are noted. Follow the links above closely. They really do help.

The original DS-3025 is not and should not be taken at the POE.

The interviewing officer at the embassy (at your k-1 interview) is supposed to return it to you, not include it in the sealed package.

I'm not sure what does go in the sealed package but I assume either they copy of or tick it off on a checklist. But the original is yours to keep

I used a copy of the DS-3025. It was in my previously sealed envelope, along with a CD. I keep anything original and send in copies.

The original DS-3025 should not be given to the K-1 applicant. It would/should be with the rest of the medical forms and included in the sealed envelope you hand over at your POE so it can be sent to the USCIS and included in your A file. The sealed envelope contains your entire file of documents for your case.

A copy of the DS-3025 is typically given to the K-1 applicant after their medical. You can include a copy of your copy with the I-485, just as a reminder and statement that your required vaccinations are complete.

When is the sealed envelope with the CD gonna be requested or something? They didnt take it at my POE nor was requested by any authority so when are we allowed to take a peek? Lol just kidding, but seriously, is it ever gonna be checked or can i stop worrying about it now?

If you were given a separate envelope by the panel physician at your medical, then it is yours. The CD is of your xray and belongs to you. It is never needed.

Send a copy to show you had all the required vaccinations. It is just kind of a reminder/flag to the adjudicator that you met the vaccination requirement for AOS.
