- Inquiry is about how to complete initial K-1 application, not a progress report -

Eighteen years in the US and I still don't understand Velveeta, TV ads for prescription drugs, only getting 2 weeks paid vacation, or why anyone believes anything they see on Fox "News".

I am assuming you are talking about starting the process and filling out the I-129F petition and not the actual K-1 visa application. They are two different things/processes.

You should list your name, and your children's names, as whatever you presently and legally go by. If you were never married before, then you do not have an ex-spouse.

Answer to that question is the answer to this question: Did YOU use the fathers name, did you have ID/passport or anything with the father's surname as your surname?

It is fine for the children to have the father's name on their BC, there is no issue with them having different surnames from you on the I-129F.
