Ive heard some people say even if you're a K-1 entering after having a medical exam within a year that you still need to see a civil surgeon to basically sign off a DS 3025 verifying you've had your vaccinations. I've also heard others say it's not needed if you've had the medical exam within a year. I read the I-693 instructions and it seemed too cryptic. Does anyone know if he needs to see a civil surgeon or is what's needed already been sent over in the sealed packet that he handed to customs?

Also if someone could move this topic to the correct forum it'd be appreciated.

In summary, the K1 visa holder doesn't need to see a CS if the medical is completed within the year prior to the I-485 filing, and all required vaccinations were given and documented on the DS-3025.

Are you sure? The DS-3025 is just the sheet from the medical with all the tick boxes for the vaccinations. I was given it at the end of my medical.

If you don't have that then you will need to go to a civil surgeon with the I-693 form.

He got one. Knightsbridge always gives a photocopy. Search any papers he brought.

The original is in his sealed package and will do just fine because they want to see the one that has remained in a chain of custody. However, without looking at his photocopy it will be hard for you to evaluate if it was completed properly or not. I am pretty sure I remember he had all the required shots plus extras he didn't need before his exam. Right? In that case you can feel pretty confident that his DS-3025 is sufficient to clear him on immunizations. Unless they lose his whole sealed envelope, you won't need a civil surgeon.

Yeah he remembers the receptionist saying all vaccinations were complete. Yes and he did get the ones needed and extras. So will we need the photocopy for AOS or are we fine without it?

The Nurse at Knightsbridge gave me a photocopy of the completed vaccination form with a stapled notice to it that reiterated her message to keep it safe, it can be used for AOS and as proof for other things i.e. school admission or employers asking for proof of vaccinations. I submitted a copy of this document with my AOS application.

I had all vaccinations done & met the requirement but in my DS-3025 It was marked "Vaccination History Incomplete" and "Applicant may be eligible for blanket waiver(s); So I went to a CS & got a I-693.

I had all vaccinations done & met the requirement but in my DS-3025 It was marked "Vaccination History Incomplete" and "Applicant may be eligible for blanket waiver(s); So I went to a CS & got a I-693.

I went through knightsbridge in London, had the same on my DS3025 as rshree and just sent a photocopy of the copy of the DS3025. I have just completed AOS and my green card has just been posted without having to go to a CS and get a I-693.

He said he did get it but somehow it's lost. We do still have his vaccination records from his clinic. Can we just bring this paper to a Civil Surgeon and have them do a DS 3025?

Most submit a photocopy of their photocopy (never send off your only one). However, that serves more as a reminder that you did indeed have shots and are using that instead of an I-693.

The real original official DS-3025 is in the envelope turned over at POE. It will be placed in your AOS folder along with the other pages of the medical report. DS-3025 is just one page of the medical report. See all the forms here http://www.visajourney.com/downloads/Medical_Forms.pdf

You do NOT have to supply a photocopy of the DS-3025. USCIS will have the original to see that his shots are fully done.

Okay. I read the I-693 instructions and they seemed very confusing seeming to imply that you absolutely needed to submit it for AOS. Also the person who gave him the sheet said "keep it safe".

I have called a handful of civil surgeons already and they all say they will only sign off on vaccinations if he takes the medical exam again..

Shop around, most say that because of the extra $$$.

Try contacting the clinic in England, see if they can't send you a copy of the vaccination sheet. Then, if you have all required vaccination, just send that. There is a whole thread about the medical on the AOS area, you might want to check it out.
