What to ask for from medical

Hello,

I remember reading somewhere that after the medical you should ask for a copy of something from the doctor that can be used in the AoS process.

I think it was something that would certify you've had your vaccinations.

Is this still the case and if so what would they call it and who should I ask?

Have mine tomorrow afternoon, nervous but otherwise ready!

Thanks in advance!

They routinely give you a photocopy of the DS-3025 vaccination worksheet. You shouldn't need to ask.

Hmm, I might have screwed up a bit

Everything went fine, except for I turned down the Flu Vaccination and now the DS-3025 states incomplete.

My thinking was I wont be applying for AoS until April, so the vaccination would not be requried at that point.

Do you think this will be a problem ?

If the adjudicator picks up your case to evaluate when it is not flu season, then he "should" approve without the flu shot if it is not flu season. That's what the handbook says. It's not when you apply, but after the case has sat there in a box somewhere for months, the date it finally gets evaluated for approval or at interview. So if it makes it before next Oct 1, you could get lucky.

The reverse...for those who wonder if it was not flu season at your medical so your DS-3025 has the waiver "not flu season" marked-- if that DS-3025 was absolutely complete when it was signed, then even if your AOS happens in winter, they go by the date of the form to check your completeness. They wouldn't say...oops, it's flu season now and send you for a new medical form.

No guarantee any given adjudicator has studied their handbook, but that is in there. The examples they use are flu season and aging out. An 18 yr old needs a Hep shot. If he didn't get one, but is 19 when his AOS is evaluated or interviewed, then as a 19 year old the Hep is no longer required. As long as the other requirements are in place, he isn't sent for a shot he needed at age 18 because he has aged out of that requirement.

Ok thanks. I was worried they'd simply use the box at the bottom as a summary and not look at the rest. I guess it wouldn't do any harm to include a cover note with the form to explain the discrepancy

Thanks for all the help

Explain the discrepancy to me. What does the box at the bottom say?

Oooo I just reread it again with a calmer minder and we might be in the clear

Here is a piccy of it

I was worried that it "2 results" isnt "vaccine history complete..."

Looking again now it seems I actually may be in the clear

If you read the Technical Instructions for Panel Physicians, it tells the doc that almost every results part will be filled out Vaccine history incomplete AND may be eligible for blanket waiver. It does not tell the Knightsbridge nurse to write all over it what you chose to do. :(

But I have known people who went with their DS-3025 with all that kind of stuff ad-libbed by the nurse who got back to me to say their AOS was approved without an I-693. I would tend to avoid explaining and hope your AOS happens before next flu season. You did it wrong but it may turn out fine without the extra expense.
