I-693 form and cost of civil surgeon fee???

I decided to visit 2 local Dr's on the list

1st one----the receptionist tells me $250.00..........!!!

I said----we dont need a physical....she said....well, with blood tests and all.....

I said ....we DONT NEED blood test

she just shrugged her shoulders and said 250

crooks

DR #2

I told them the same thing---no physical/blood tests......

"well, the Dr cannot just fill it out without seeing her"...she says

$200.00

I have maybe 4 more near me

Looks like I will just have to bend over and take it like a man....LOL

Any other experiences out there.

I absolutely HATE getting hosed

Same thing happened to me... I finally found one that was willing to do just the paperwork for $90. Just keep trying...

Hi bgamb,

You posted a similar question yesterday and received replies. I mentioned that we did not go to a CS. We included our DS3025 and this portion of the instructions from USCIS:

3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

Our interview is November 26th. No RFE.

My new wife had NO record of immunizations in the past

So, the only thing she got was what they decided in Manila at St Lukes

I am not real clear when I read her DS-3025....is it complete and acceptable or is there anything more she needs medically

I believe she got 4 shots (I can't get her on the phone now to confirm)

The column "insufficient time interval" is what concerns me

I assume that means there was not enough time for a "series" of shots

So......is she required to have more? therefore the NEED for the civil surgeon?

If you look at the official link that steve posted....paragraph 4d2 states even tho a medical exam is not required....the form must be filled out to prove the vaccinations are done.

And this must be done by the civil surgeon

It seems clear that all these "dr's" that want $250-$300 are scamming us just to fill their pockets

we just need a sign off that proves that the ds-3025 was done

I hate getting hosed.....

I will keep looking....but once the distance to a doctor increases...it almost becomes not worth the savings

You have a copy of the DS-3025 properly completed so you don't need to do the I-693... Mine wasn't properly completed back in my home country so I had to get the I-693.

It was frustrating for me too to find someone who wouldn't want 300 dollars for everythig....

Hi all.....

Once again I appeal to your experience in the matter of filing my wife's AOS form 485.

This is about the medical form 693.

My wife had her physical and her vaccinations at St. Lukes in Manila. We have her DS3025 form with the record of the vaccinations with us.

So...do I need to fill out the 693 form? Do I need to have a Dr. do it? In my part of the country (northern NY state) they're a little hard to come by and since she's not on my insurance yet, it'll be out of pocket costs.

I've read some posts that stated that they didn't have a Dr. do it and they were all right with this when they sent it in.

Please...guidance??

As for the people above, I feel your pain. Some civil surgeons really do take advantage of us intending migrants. I unfortunately did not have complete vaccination records. So was charged an exorbitant amount to receive the vaccinations and medical exam. In the price range of $400 upwards.

I called a CS today and explained to them what I needed and they tols me to come over tomorrow (Thurs.) and she'd fill it out for $25. Now, this is class. She was up on everything and NOT trying to cheat people out of their money. Frankly, I think the USCIS should take the approved doctors and straighten them out and explain that NOT everyone needs to have another physical. To charge upwards of $300 for this service is the leading cause as to why medical care is not within reach of many people and insurance costs so much. OH WOW! I'm very happy to hear what that CS is able to do for you. She is definitely the minority, but it restores my faith that there are still good and honest people out there.
