DS-3025 Rotavirus row blank

Greetings everyone!

My wife (who came here on a K1 fiance visa from the Philippines) is ready to apply for AOS. To that effect this website has been great for all of the wonderful information that is gathered! I did however have one question that I can't seem to find an answer to on any of the posts that I've been able to look through (is there any sort of search feature on these forums that I'm overlooking?).

My wife had here medical over in the Philippines and everything appears to be filled out correctly on her DS-3025 so that we can just submit that instead of an I-693 (she had some not age appropriates and at the bottom is marked history incomplete - may be eligible for blanket waivers which from what I've read is fine). The one thing though is in the rotavirus row, it is completely blank with no check mark or anything. Does anyone have any experience with this or know if we now need to get a civil surgeon involved? It seems like this is common for this row to be blank at least on Philippine forms but just wanted to get some more opinions so we don't kick ourselves later if we end up getting RFE'd on it.

I would personally encourage you to see a civil surgeon if you wanted to have a smooth flow on your AOS process. I recently got an RFE because i didnt transcribe my DS-3025 to I-693 because i also thought it would be ok since it says may be eligible for blanket waiver but i didnt had the luck and ended up getting an RFE requesting for the transcribed form (I-693) and got flu shot since its already flu season but be sure to look for a civil surgeon who wont require you to redo the medical exam but will just transcribe your vaccination sheet

( DS-3025) to I-693 their prices ranges from $35 - $100 for transcribing alone.

Depending on your where you live try to call as many civil surgeons as you can and compare their prices and their requirements. Here's the link so you can locate one closest to you

Thank you very much for the response! I will take your advice and check into a civil surgeon, especially since it seems like this whole submitting the DS-3025 is like a roll of the dice.

I would ask them to fill out something in the rotavirus section. My husband and I are going through AOS and just got an RFE saying his application was incomplete because it did not indicate that he has received the rotavirus vaccine. It says:

USCIS notes you submitted a copy of your Vaccine Documentation Worksheet; however, your Vaccine Documentation Worksheet does not show you received the vaccine for Rotavirus. Your Vaccine Documentation Worksheet does not show you a waiver of the vaccine for Rotavirus. Please submit a completed Vaccine Documentation Worksheet with Form I-693.

We obviously do not want to pay to see another Civil Surgeon. I am thinking of having our regular doctor write out an explanation that Rotavirus vaccine is given to children at 2, 4, and 6 months of age so my 27 year old husband does not need it. I was also going to print the documents from the CDC website verifying that rotavirus is for children and is not recommended for adults. I hope it works! Bottom line, have someone fill it out before you mail it so it doesn't come back to get you later.

You can try this, but I am not sure they will accept it. The instructions state that if you do not have a properly filled-out DS-3025, then you need to see a civil surgeon to get the vaccinations transcribed onto an I-693.
