On January 8th my husband and I have an appointment with our advisor to file for AOS, except she hasn't been getting back to me in regards to any of my questions.

If I'm not misunderstanding - my husband needs to go to a civil surgeon? For vaccines?

I would like to know what that is all about. Where, why, how, what do I need from the doctor? How should I go about making the appt, etc.

Any help would be appreciated!

In this game is important to be proactive. Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy).

He will not need to get a whole new medical, just the missing vaccinations(let them know this when you call-it is stated in the instructions for the I-485 and I-693). However, it can be a pain to find a civil surgeon that will not insist on a medical and/or overcharge you. My husband was given a blank DS-3025 at his medical. We had to call around to several civil surgeons before we found one that would do the vaccination transcription only. He had all the required vaccinations already except for the flu shot(bring any vaccination records with you), which he got there for $35. We were charged another $25 for the vaccination transcription onto the I-693. We had to drive 2 hours away to find this civil surgeon. It can be a pain finding one that won't insist on a new medical or overcharge you. There is a thread here on VJ for civil surgeons who don't rip you off. The required vaccinations for AOS are at least one in a series for MMR and Tdap/TD, varicella (unless he already had the chickenpox), and the flu shot(only during flu season Oct. - Mar.) http://www.cdc.gov/immigrantrefugeehealth/exams/ti/civil/vaccination-civil-technical-instructions.html#tbl1

She also had the physical that is given in Ciudad Juarez prior to the K-1 interview. Once she arrived in the US, we did have a difficult time finding a civil surgeon that would give the vaccinations without also doing a physical. I wanted to get this done ASAP so we could apply for AOS, so we ended up paying for another physical just so we could get it all done and over with. It was expensive, but there was some piece of mind in knowing that it was all done quickly and properly. Quotes varied by as much as $150.00 from one civil surgeon to the next so it is good to call a bunch and see how much they charge for this service.

My husband has ALL his vaccines (and proof!) his flu shot which I made him get upon arrival here a month ago, AND his physical from Juarez less than a month ago.

I just need the transcript. I literally just copied and pasted about 30 office numbers/addresses and I'm calling each one (or until one convinces me) that they won't rip me off.

I've gotten so far ranges between $80-200 just for transcript and doctors who want to shove a physical down our throats. No way.
