I am getting my fiancee AOS paperwork ready after our November 24, 2012 wedding. On Form G325 in the first cell it asks for her family name. Is that her now married last name or should I put her maiden last name there?

She can put her new married last name (if she wishes to use her married last name from now on). In the field "All Other Names Used" she has to put her maiden last name (of course in case she put her married name in the first field).

All the AOS forms has to include the same last name (married or maiden).

Fill out all the AOS forms in the name she will legally be using from now on. If she is taking your last name from now on, then that is what you use. Her EAD/AP and green card will come in the name you use on the forms. The copy of the certified marriage certificate you send in with the AOS paperwork acts as the legal name change document.
