A full medical is not required, but if her vaccination requirements were incomplete at the time she got her medical in her home-country, she will need to see a Civil Surgeon in the US and have the required vaccines done.

Does she have a copy of her vaccination record from her medical?

It is my understanding that NO you do have another medical examination, but that the civil surgeon has to look at your vaccinations to make sure that they are current. He only has to take your vaccination sheet, and complete an additional form that says all vaccinations are complete, no further ..... You will not be charged for a medical exam.

If I am wrong, please clarify.

You need to know if she met the vaccinations requirements at the time of her K-1 medical (they were not mandatory to have the K-1 visa, but they are for AOS). If yes, then no problem, even if it's better to include a copy of her vaccination record DS-3025 in the package (but you don't have it). The original vaccination record DS-3025 should be in the sealed envelope that she gave at POE.

If she didn't meet the vaccination requirements at the time of her K-1 medical, she needs to have the missing vaccines done, and have a civil surgeon fill in part I of the I-693 + the vaccination record page.

It's all written in the USCIS instructions of form I-693.

"1. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

2. Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and

included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693."
