Hi Everyone,

I know this question has probably been asked many times before, but Im a little confused. We have our NOA2 letter, A number and LND number. Instead of me receiving my next instruction, I received a letter stating that the embassy had not heard from me...so I called the embassy and they stated that the letter was asking me to fill out the following forms: DS230I, DS156, DS157, IV15 and I134. My question is this, do all forms need to be sent at the same time, or can I send some and take the affidavit of support with me to the Embassy, and when can I book the medical, and do I take the police cert with me or send it with the DS230 form?

Sorry if this sounds silly, but I dont have the letter to refer to, and the Embassy were unable to send a duplicate?!?!

What needs to be sent to the Embassy is:

DS-230 Part 1

DS-156 x2

DS-156K

DS-157 x2

If you have all the required documents needed for the visa interview (see checklist on US Embassy webiste), then you can also send form DS-2001 and a covering letter in at the same time (example of covering letter can be found somewhere).

Also since you have your LND case number then I would call and book the medical asap. What you need for medical is:

Passport

Passport sized photograph (can be UK or US Size)

Police Certificate plus copy

Copy of Immunization records

Medical questionnaire

Thanks for the information here guys...whilst on the phone to the embassy, the advisor also stated that I need to fill out form IV-15 - however I cant find this form listed - any thoughts??

They were likely referring to the IV_15 page on the Embassy site, which lists the forms mentioned upthread: http://london.usembassy.gov/iv_15.html

That makes perfect sense now I see it on a page...I've got all of the info now, thanks everyone!!

Would have been easier if the letter could have been re-sent, but nevermind visajourney helps again \o/ yay :)

IV-15 was the form number of the old London checklist from when they actually mailed out a packet of instructions, forms, and a list of documents to bring to the embassy. The "checklist" had places to tick off the documents you had in hand prepared to bring to interview

Like this:

___ police certificate

___ affidavit of support

___ divorce decree

(etc)

It also had a place to fill out date of medical exam, travel, and marriage. And you signed that you had all those documents and were prepared to interview.

Since 2009ish, the IV-15 has been replaced by the DS-2001 form to certify you have all your documents in hand but you don't physically put checkmarks on a list anymore. They ask for a cover letter to include the dates requested on the old form. Don't send it back until you have all the documents needed to take to the interview. It can be mailed separate from the four application forms after you finish gathering your documents.
