Hi Guys,

I've been hearing some people talking about the UK Birth certificate being "Long Form" and "Short Form" and being required to have both... Can anyone give me/tell me where there is some more info on this topic?

Thanks in advance!

http://london.usembassy.gov/document_checklist.html#birth_certificate

Birth Certificate: Obtain the original, or certified copy, of the birth record of each family member (yourself, your spouse, and all unmarried children under the age of 21, even if they are not immigrating with you. If the children are deceased, so state giving year of death). The certificate must contain the following:

Person's date of birth;

Names of both parents; and

Annotation by the appropriate authority indicating that it is an extract from the official records.

Note: We will not accept the short form version of a UK birth certificate. The certificate must list both parents'names.

The birth certificate has to have the parents name on it. Some Uk people have a "short form" they used for school without the full details of the parents...just name and birthdate of the person that the certificate belongs to. That is not accepted.

Here's an example of the long form birth certificate

Wow, my husband's looks a lot different. It's landscape mode and truly "long" meaning it has to be reduced a lot to copy to a regular sheet of paper. At first, I thought that's why it was called the "long form". LOL. Then I started reading the London website.

I guess that one pictured is what you get if you order a certified copy...they certify the information is copied from the registry.

I Just pulled out my mothers birth certificate. Yup, the same ultra wide version as you specified. The above is exactly what you get if you order a replacement though. I wonder how the consular would feel if you turned up with a sheet of A3 paper for your birth certificate.... I have a feeling the grumpy one might have a fit!

Thank you all to helping out with this post.

It turns out that from what I've read I (Ashley) have the short form of certificate. I'll be doing some research on how to apply for another one this weekend but has anyone had to apply for a long form copy and willing to share their experience?

Thanks again in advance.

Fifth post down in the NOA2 to interview thread says this

Birth, Death, Adoption and Marriage Certificates

You may as well order this when you read this but Birth Certificate replacements no longer require a trip the the Births Deaths and Marriages office. You can now order online through the General Register Office at £9.25 for a birth certificate

General Register Office

That's what I ordered mine from, and the easiest and quickest method if need be, They have an expedite service as well if you so desire. You can apply to your local council but this tends to take longer.
