I just received the approval notice for I-130 (IR-5), but it is not transferred to NVC. It states I have to file I-824, for my relative is living in the Philippines. On the I-130, number 22, I put N/A on the left box, but left the right box blank. Did I make a mistake? Can this still be corrected so I don't have to file I-824 and pay?

yep, they don't know where to send it. for adjustment of status in country you complete the left part of question 22

it say if it is going to an embassy abroad, to complete the right side. that's why it says city, country.

now you will have to pay the i824 to send it to NVC
