So, we are just about ready to copy everything, all forms are filled out, checks are written, and....

We are supposed to turn in DS-3025, Vaccination Documentation Worksheet, along with the I-485. I have always thought that the Civil Surgeon took care of this matter, but after going through the Visa packet that was received at the border, I found the DS-3025's signed, but not filled out. It from the Civil Surgeon in Ciudad Juarez. My wife and step-children did receive their vaccinations as needed, but since the Civil Surgeon here in Phoenix, sealed the I-693, I am not sure that the vaccination record is included.

What do we do now? I admit. I am in over my head on this. Of course, we have saved everything, and if needed, can spend the next few days looking for the written records for all their vaccinations.

Anyone know if the I-693 has everything included? Please someone. Help us. My step-son, who has a K-2 will be 14 in a few days, and we would like to avoid paying more. According to the immigration help line, as long as send in the packet before he is 14, we will be okay.

Actually the packet has to be received and the NOA1 generated before he's 14. Not just mailed.

The doctor in Mexico was a panel physician, rather than civil surgeon so it's less confusing. You're right the panel physician didn't fill out the DS-3025. That's standard with Mexico. The CS here in the US should have completed the vaccination portion of the I-693. You should also have received a copy of it.

I will look for the copy of the I-693, but will we have problems if we just send in the sealed envelope without the DS-3025's that we were given at the border? And, to clarify, the first NOA is just that they have received our application. Is that right?

Aren't you the person who has waited many many years to file AOS? If you got a new medical (your old ones are expired) then the new medical on I-693 has everything you need. If you are another person, my apology for the mistake.

Well, we have waited a little over two years. We have received much advice from you, and have did all that you have advised. My wife and step-sons did redo their I-693's. They are less than one year old.

Oh, one more thing. Since the I-693's are sufficient, I am out the door to my local Kinkos. Thanks.

Ahhh so COMPLETE new I-693's.. not just transcriptions. Then yes the i-693's should be fine (and you should have copies).

Don't forget the I-130's!
