Surname on passport

In some parts of Asia, the surname is the first part of the name. For example, a person named TANGELLA SANDEEP REDDY may have on his passport

Surname: TANGELLA

Given name: SANDEEP REDDY

while the birth certificate and all other documents would have him listed as TANGELLA SANDEEP REDDY.

USCIS may process this case with the beneficiary name of REDDY, TANGELA (consistent with the birth certificate). But when NVC proceeses the case, they will make the last name TANGELLA because they go by surname on the passport being the same as the last name.

I understand that if the NVC uses the surname as the last name, then the visa and any other official documents (social security card, etc) will use this same surname (TANGELLA) as the last name. So in the US, his name would effective be changed to SANDEEP REDDY TANGELLA. Is this correct?

We are in this situation and it is difficult to get a new passport/ammended passport as this is the naming convention in this part of the world. Has anybody else been in a similar situation? Will this cause problems down the road? Please advise.

The same in China. Has not been a problem for us thus far. Surname remains surname and given remains given. True on the social security card (and most US forms of ID) it is reversed from her passport but it the US the surname (or family name) is usualy printed at the end of the full name.
