Does anyone have a copy of the "Interview Readiness sheet"? The one page (front and back) notice that comes with your interview appointment notice, explaining what to bring if you apply based on marriage to a USC, if you owe back taxes, if you were in the military etc. It's a printed page, no personal info on it.

I cannot find mine, must have thrown it away, and a friend was asking about it. Would appreciate it being posted here or PM me for my email addy.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.
