Another question this morning:

How do we translate the instructions for the medical examination?? K1 visa holder, Arrived less than one year ago, but I take it as a bad sign that I have not heard of the form DS-3025. I take it that this form was never filled out at the overseas examination. Does this mean that my fiance must be examined again? He doesn't have an official vaccination record either. We do have the chest xray.

I take it that this form was never filled out at the overseas examination. Does this mean that my fiance must be examined again? He doesn't have an official vaccination record either. We do have the chest xray.

Your husband will not need to get another full medical exam. However, you will still need to go to a civil surgeon to have the I-693 filled out. You can either get the required shots from the CS, or get them elsewhere cheaper and bring the records with you to the CS. You will need to have Part 1, Part 2-vaccinations section only, and Part 5 filled out on the I-693. You fill Part 1, and the CS does Part 2 and 5, then seals it up in an envelope for you to send in with your AOS paperwork. You will most likely need to call around until you find a CS that will not insist of the full medical and higher cost. It should only cost about $50 or less to get the form filled out without the medical exam. You can print out the instructions for the I-693 and bring it with you, in case you need to show them proof that the medical is not needed for someone adjusting from a K-1.

I found the DS-3025! It was inside the large envelope with the x-rays.

The instructions for the 693 state that if you are a K non-immigrant then a) you don't need another exam as long as there were no Class A medical conditions found or if they were resolved and b) that we still have to show vaccination records. Does the DS-3025 satisfy the vaccination requirement? Or do I need to get the vaccination schedule in the 693 signed by the Civil Surgeon.

I called a surgeon and he was kind of a jerk saying he didn't know what he could do for me since i read him the list of vaccines my husband had taken in March and he said that my husband already had the required vaccinations. Then he told me hoped i was using a lawyer because i didn't know what i was talking about when i told him we didn't need the full physical examination. oy!

The 485 instructions are not very clear either stating that if you are a K-1 fiance within the last year you don't need a physical but you DO need to submit the vaccination supplement, again not clear if the DS-3025 satisfies, or if it has to be the 693.

If his DS-3025 is complete, meaning all the required vaccinations are listed there as him getting them done already, then you only need to send in a copy of the DS-3025. No need to get the I-693 done in that case.

That doctor was silly. You don't need a lawyer, or another medical exam. :P

Any ol' DS-3025 may not be acceptable. They'll give you a DS-3025 whether you got the required shots needed for AOS or not. Some doctors fail to sign them. Some fail to complete the results section. .

There are certain things your DS-3025 should have to be considered complete.

Look at your DS-3025. If it has the following items, you don't need an I-693.

1. Shots marked (at least one of a series)

  • MMR
  • Td or Tdap or DT or DTP or DtaP (date no longer than 10 years ago)
  • Varicella or VH written by it if you had chickenpox

2. Everything else marked not age appropriate or medically appropriate.

3. RESULTS section filled in with "incomplete". AND "may be eligible for blanket waiver"

4. Signed and dated by the doctor

If it's not completed like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025.

The USCIS is supposed to have your medical files, but sometimes lose them or fail to match them up with your AOS. Do not get another exam unless you get an RFE that says they do not have results of your medical. It means they lost them, but you are probably stuck with paying for a new exam. Your official vaccination results are also part of that file. We send the DS-3025 photocopy more to remind the person opening the mail that we are intending to use that DS3025 to prove our vaccinations, rather than an I-693. The official "sealed" version is in you A-file (brown envelope) which they are supposed to get and match up with your AOS.

The DS3025 has to have all those things, not just some of them. Incomplete is fine if the box about blanket waiver is also ticked.

Your original DS-3025 was in the envelope you turned in at POE with the rest of your medical results. Sending in a photocopy reminds them you are using that as your proof of meeting the requirements without an I-693.
