AOS / I-864 and IV / DS-230 checklist

Hi everyone. My husband and I received an email this afternoon from the NVC which stated that they received all the fee payments and that it's time to send the documents. I'm anxious! :unsure: Per suggestion from one of my friends, I should post our checklist and hopefully you guys can check it also. Any suggestions and corrections are very much appreciated. Thanks very much! :)

AOS package

- bar-coded cover sheet

- personal cover sheet listing all the documents submitted

- petitioner's I-864

- petitioner's W2 and 1040 for the latest 3 years

- petitioner's proof of active duty and copy of re-enlistment paper in lieu of certificate of employment

- petitioner's copy of military ID

- petitioner's LES for one year in lieu of pay stubs

- co-sponsor's I-864

- co-sponsor's W2 and 1040 for the latest 3 years

- co-sponsor's letter of employment

- co-sponsor's pay stubs for the latest 12 months

- co-sponsor's copy of green card (front and back)

IV package

- bar-coded cover sheet

- personal cover sheet listing all the documents submitted

- beneficiary's DS-230 part 1 signed, part 2 unsigned

- beneficiary's 2 passport-size pictures: glossy with white background

- beneficiary's NSO birth certificate printed on security paper

- petitioner and beneficiary's NSO marriage certificate printed on security paper

- beneficiary's NBI clearance

- beneficiary's copy of biographic data page of passport

- 1 set photocopy of each of the above documents

Forms were filled-up using CAPITAL letters.

According to reciprocity by country, we should use NBI clearance in liue of police certificate.

CENMAR ( Advisory On Marriages ) = if you have this might as well send one

Hi. Thank you for the reply. I got this information from travel.state.gov so I guess I don't need to send my husband's birth certificate:

Who needs to submit petitioner documents?

If you are applying for an IR5 (Parent of a U.S. Citizen) visa, you are required to provide:

1.Original or certified copy of your Petitioner’s birth certificate

2.Original or certified copy of your Petitioner’s marriage certificate from the Petitioner’s current and all previous marriages.

If you are applying for a F4 (Brother or Sister of a U.S. Citizen) visa you are required to provide:

1.Original or certified copy of your Petitioners birth certificate.

About the CEMAR, I believe we'll bring it on the interview.

I also got the email from NVC yesterday that case number should be written on the upper right portion of each document. :)

12-07-2010 Received NOA2 hard copy

NOA1 to NOA2 - 43 DAYS

NVC:

12-09-2010 NVC receives case

12-10-2010 attained NVC case# & IIN. PAID AOS FEES. emailed request for expedite & choice of agent

12-13-2010 AOS PAID

12-16-2010 received email granting the expedite request

12-17-2010 NVC sent case to US Embassy in Manila. SIGN IN FAILED! CASE COMPLETE!

Hello! The medical fee at St. Luke's Extension Clinic depends on the current USD-Php exchange rate for that day. I had the medical exam last Wednesday and I paid Php 9,387.50. The fee payment must be in Philippine peso currency. SLEC won't accept credit cards, health insurance or any other foreign money. Goodluck! :)

All foreign documents need to be translated to English by an authorized translator. I didn't have my documents translated because civil documents here in the Philippines are already written in English.

I'm at the NVC stge as well and I'm pretty sure any document that you have to submit to USCIS or NVC needs to be translated to English unless if they're already written in English. I advice you to do the translation rather than getting a checklist/RFE from the NVC because you failed to have your documents translated. That way you can avoid further delay.
