I really-really hope my question is not that much weird but...

what family name (last name) should be used in I-693 form? My maiden name or my last name after marriage?

because my 3025 form (issued in July'10) has my maiden name

If your I-693 was completed before your marriage, then use your maiden name. If the I-693 was completed after your marriage, use your married name. You will be submitting a copy of your marriage certificate with the application plus you are listing both names on the application so they will be able to figure it out.:)
