Need help with filling out the DS-156 Form

Hello Everyone,

I am in the process of filling out the DS-156 form for my fiance. However there are a few questions, which I need some help with.

18). Spouse's Full Name

(Even if divorced or separated, include maiden name)

Both of us have never been married before. In that case do I leave this space blank or just say "None".

19). Spouse's DOB (dd-month-yyyy) -

Can I leave this space blank as well.

22). When do you intend to arrive in the U.S.? I am hoping if all goes well by June 15th, 2010. Me and my fiance will travel to the U.S. together. Can I put that date in.

26). How long do you intend to stay in the U.S.?

WHat is the appropriate answer for this!

27). What is the purpose of your trip?

28). Who will pay for your trip?

Me - Petitioner.

What type of Visa? This will be her first time coming to the U.S.A. What is the appropriate answer to select in that space.

32). Do you intend to work in the U.S.? "Yes" after she gets the green card, however there is a section underneath asking for name and complete address of U.S. employer. There is none at this point

can I just write "None".

33). Do you intend to study in the U.S.? "Yes", however at the moment school is not decided. What can I enter for school name and address or should I say "Not decided at this point".

27). What is the purpose of your trip? marriage and relocation to the USA

What type of Visa? This will be her first time coming to the U.S.A. What is the appropriate answer to select in that space. nothing, because the questions is: have you ever been issued a US Visa you have to select NO

32). Do you intend to work in the U.S.? "Yes" after she gets the green card, however there is a section underneath asking for name and complete address of U.S. employer. There is none at this point

can I just write "None". with work authorization, employer unknow.

33). Do you intend to study in the U.S.? "Yes", however at the moment school is not decided. What can I enter for school name and address or should I say "Not decided at this point". I suggest select NO
