Comments : as others have said - don't wait for the Pkt 4 to arrive. Ours came after we had already been approved for the visa. Had we waited it would have been two more months. Simply check the consulate website for your case # and as soon as it shows up there you're good to go. Download the packet, get your medical done, and schedule the interview.
