Hi Menuka, for the medical, you should continue calling the Institute of Migration (IoM) Nepal to schedule your appointment. Typically, you would want to get the medical appointment done within 5-7 days before your interview. The results are provided within the next day.

We scheduled our medical appointment 19 days before the interview. The time between the medical appointment and the interview was 6 days. Then, we went to pick up the results 5 days before the interview. Keep calling them and let them know about your upcoming interview. Sometimes, it just takes time and the right person to schedule your medical appointment. In the meantime, start preparing the documents and things you need to take to the medical appointment AND the interview. Let me know if you have other questions.

So on 2020 W2 I don't have enough income coz I was out of country the whole year. I have enough income on 2018,2019 and I'm making 32k now . Do I need joint sponsor? I'm new I don't know much about it . So

It depends on the size of your household. In this case, how many people, including yourself, do you count when filing your taxes currently, AND how many people you are petitioning for K-1/K-2.
According to the new Federal Poverty Level Guidelines, assuming it's you and your beneficiary (2 people), 125% of the New Poverty Guideline Levels for 2 people is $22,887. Keep in mind that your joint sponsor also needs to show their income level at or above 125%. Resource Link Below.
As for the missing 2020 W2, I am not sure what you could do there. I would try these methods to make your case strong by providing enough evidence that you can support your beneficiary through the time they are not able to legally work. I would definitely have your beneficiary take all of your 2021, 2018, 2019 W2s. Also, have your beneficiary take the Tax Return Transcripts for the last 3 years if you can; I got these online through IRS (you need an account to download them). Resource Link Below.
Make sure to take all of the Supporting Evidence required for your Form I-134. Resource Link Below.
Lastly, write a short letter explaining your situation as to why you were not able to earn enough income in 2020.
