Hello! I had the medical appointment on June 29th and 8 days later the Mater Private called me to collect the package. I submitted the notification of readiness right away and today the embassy emailed me the Visa interview date with further instructions including the link to register the appointment and pay the Visa application fee (MRV). The booking process went pretty smooth, I paid both for MRV and delivery at home service (for the passport); be aware that every time you make a payment you will have to wait some time for their system to receive and process the payment (no more than 1 hour in my case). You will receive the email when the payment has been processed and you will then be able to continue and finish your Visa appointment registration.

Hello! Yes, at the interview they will ask you for the affidavit of support, make sure to bring one copy and maybe print a second copy for your records.
