The visa was ready for delivery on the 5th, but we only received notice that it had been mailed 20 minutes before the courier attempted to deliver it, and we missed the delivery as a result. The 6th was a holiday in Italy, so we received the visa on the 7th.

No. We haven't requested my wife's case to be transferred out of Montreal yet because we are waiting for her Canadian work permit to be renewed. (We wouldn't want her to leave the country and suddenly lose the ability to work while she is preparing to move to the US.) And Montreal is moving painfully slow.

Thanks! Not going to lie, it was weird. My wife was told that she was approved on the spot, but also that the consulate was having trouble accessing the results of her medical exam, and that there would be a delay "of a couple of days" as a result. On CEAC her status is still at "Ready," so we're not celebrating yet, and we have no idea when she'll get her passport back. This might be a State Department-wide issue; some people in Montreal are currently complaining of the Montreal consulate not having received their medical exam results either. I was aware of that issue in Montreal before my wife went to the exam and so I suggested she insist at the diagnostic clinic that she be given a hard copy of the medical exam as a backup, but they refused.
