You get an e-mail saying that you can choose your appointment date. They are only having interviews on tuesdays and thursdays for K1 visa filers. My first available option was in about two weeks, but I scheduled my interview in three weeks from the day that I could choose my appointment date. I scheduled my medical 10 days before my interview, but because they just switched to another Medical clinic, it took a lot longer for the consulate to get the medical results (I think in total it took about 4 weeks for the consulate to get the medical results from the clinic).
Scheduling the medical is really easy. You'll get an e-mail adress from the medical clinic and you can just e-mail them for a date (I think they're also having appointments on tuesdays and thursdays only). It's possible that they ask you to get some vaccinations beforehand (I had to get 2 vaccinations at the doctor).
