Issues After the Fact:
1.
They entered my fiancee's First name from her India Passport as First & Middle name. So when we went to SSA, the name did not match in database and it took a little longer as SSA had to update S.A.V.E. Database.
This was not much of a problem, but word of advice is when POE CBP officer enters your information into the database from your passport, make sure to give them a heads up about your name, First Middle and Last.
2.
The entry stamp on passport said Entry on June 30, 2014 and Expiry on September 29, 2014, which did not match the Admit Until Date on I-94, which said September 27, 2014. SSA had trouble with this and said to go to the nearest POE and have them change it. We went to nearest POE airport and the CBP officer just changed the 29 to 27 with a pen on the passport, we could have done that!

I did not include I-693, instead I submitted a copy of Vaccination Document DS-3025.
Nov 5, 2014: "Your I485, APPLICATION TO REGISTER PERMANENT RESIDENCE OR TO ADJUST STATUS is now ready for interview scheduling."
"On November 14, 2014, we scheduled you for your interview and mailed you an interview notice for Receipt Number MSCxxxxxxxxxx. Your interview is on December 19, 2014."
The officer in the interview wanted the vaccination record in I-693 format, he thought something was missing/wrong in the DS-3025 vacc. form from India. So I went to a Civil Surgeon, paid $75, and got it translated from DS-3025 to I-693. I submitted the I-693 (just the first page and last page which is the vacc. part), waiting approval.
"On January 5, 2015, we ordered your new card for Receipt Number MSCxxxxxxxxxx, and will mail it to the address you gave us."
Received the card in the mail on Jan 10th, finally! Thank you VJ for ALL THE HELP!!! |

Used stamp to travel to India (Los Angeles -> Abu Dhabi -> Mumbai), Etihad took it without issues.
Initially tried taking Virgin Atlantic flight from Los Angeles -> London -> Mumbai but we were denied at LAX airport because Virgin Atlantic said London does not accept the green card STAMP, they only accept the physical card (ours was expired and the extension in NOA had expired as well).
03/19/18 : Approval Notice I-797 (states I should receive green card in mail within 60 days)

Though at the time of the notice, USCIS online case status is still not updated, showing last update of "October 14, 2016," when they received my case

04/03/18 : Received email saying green card has been mailed:
