We emailed NVC about the error and was later assigned a new "TKY" (Tokyo Embassy) case number. A neighbor dropped off the IV Bill as the Post Office delivered it to the wrong address. NVC says DS230 form was generated/mailed on March 24th, but...it's lost mail? NVC says I have to wait 4 weeks before requesting another copy to be mailed out. Update: it wasn't lost mail, NVC just never mailed it before and held it for 3 1/2 weeks! After Case Completed at NVC the process moved very quickly and easily. for the medical exam we used the Tokyo British Clinic. no problems there. the interview day was memorable due to a long wait and evacuation at the tokyo embassy due to a bomb scare. Received conditional green card in the mail 3 weeks after entry. Social Security did not have any record of us applying for a number from the DS230, so we had to go to our local office to apply again. 2010-09-08
