My wife went through the interview process at the embassy in May 2010 and overall she said was a comfortable experience. Their office was quite responsive (1 day response time on two separate occasions) to the following email address for us: Congensansal@state.gov. They responded to our emails and delivered both packet 3 and packet 4 via email.
The only roadbump we had was figuring out how to send the packet 3 materials via cargo expreso, as they mandate. Good luck trying to find their phone number or address using a phone book in El Salvador or anywhere on the Internet. My wife finally found by going to the embassy and there was a small sign there that had directions on how to find cargo expreso (sorry can't be of more help, as she doesn't remember the address).
Interviews went smooth. First one was just handing in the paperwork / medical exams from packet 4. 2nd interview scheduled one week later where they asked more detailed questions about our relationship, but my wife felt comfortable and didn't run into any issues. They approved her that day.
Note that upon approval, they kept her passport and said would send to her via cargo expreso. Turned out taking about 3 weeks for that process, so be careful scheduling a flight...allow 4 weeks to be safe.
