My fiance arrived at the consulate at 6:20AM. At 7:00 AM he was asked to join the line. His appointment confirmation letter was checked outside before entering (be sure to write your ACC case number at the top).

Security checks your passport matches the confirmation letter. When you go inside there is a security checkpoint- to enter the embassy. When he entered he gave a copy of the D160 and had a seat. They then called his name to take fingerprints, passport photos, birth certificate and police report. He didn't have a copy of his birth certificate- so they made a copy of it and charged $1USD. At this point a woman asked: "Who filed for you?" and "Where are you from?".

He sat back down and was called back after 15 minutes- to a counter to talk to the counselor.
She asked:
What is the name of your fiance?
When and how did you meet?
Has she been married?
Does she have kids?
Do you have any wedding plans?
When and how many times did she visit?
What does your fiance do for a living?

He then sat down and waited for 50 more minutes. He was then called into a private room and asked if they should go ahead and approve his visa... and of course he said yes!

Note: None of the additional documents that he brought were checked! Everyone he spoke to was professional. He received a paper explaining next steps to pick up the visa and was finished around 11:20 AM- the total time was 5 hours.
