My fiance wrote the following about his experiences in Madrid. He scheduled the medical for the day before the appointment, and was really impressed with how polite and nice everyone was at both the doctor's office and the embassy.

The last step of the K1 process was so easy, I had my medical appointment the day before to go to the embassy, so I could got my results before to walk to the embassy (the day after the medical inspection that was easy and the medical staff were so nice).
The only problem in the embassy was that nobody told me about to avoid to carry any bag or item like luggage or something similar so you should just bring a folder with your papers and the medical results.
I got the embassy early and they told to come back 30 minutes before the hour i had scheduled, then the officers checked my passport and made me to pass the normal controls (you have to leave your cellphone at the door and go inside the visa office).
In the office you have to take a number and save it the whole time
- the first call will be to check your documents (the ones they already have it cause you sent to them) and show the payment confirmation for the DS-160 and the Medical results.
If everything is ok you will receive an envelope with the X-RAY medical examintions (you must save it and bring it with you in your handbag when you fly to the USA).
-The second call (important to save your number) will be to talk to the Consular Agent (mine was a really nice lady who spoke in Spanish and asked me the normal questions)
Those questions were about how I meet my future wife, how does I like her family and if I meet them, and if I had been in the USA (one of the guys who was waiting with me never being in the USA before (Oh boy) and he got the visa anyway so that is so good. They also asked what my girlfriend does for work in the US.
They will tell if you are approved after those questions.
My experience was really good, and the people who was in the same process I meet those days were really nice so don't have any doubt about to share your experiences with them.
