My wife had her Medical and Embassy / Consulate interview 10/21-22/2015. She told me her medical went wonderfully and the interview fast and easy. However, one point which should be pointed out for the medical portion is the Vaccination requirements. According to the website, http://travel.state.gov/content/visas/en/immigrate/vaccinations.html under Vaccinations it states:
Important Notice to Immigrant Visa Applicants Concerning Vaccination Requirements
United States immigration law requires immigrant visa applicants to obtain certain vaccinations (listed below) prior to the issuance of an immigrant visa. Panel physicians who conduct medical examinations of immigrant visa applicants are required to verify that immigrant visa applicants have met the vaccination requirements, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccination... read complete review

Quite simple.
My fiancee went in around 2:30pm and had her interview at 3:00pm. They called her for fingerprints and to hand in the necessary documents. After, she was called to another window for about a two minute interview with very simple questions. They said she was approved.
Why only 4 stars? Well, the embassy sent her visa stapled to the envelope holding the "mysterious packet." She unstapled it, opened the envelope to get the packet (which she did not open...only the envelope which it was inside.) So, because they stapled that back cover (not page, but cover) to the envelope, she has two small holes on it. This shouldn't affect anything, but it is highly unprofessional.

The medical and interview process was quite painless and rather quick. I received the appointment letter on August 31 and my interview was on September 11 at 14:30.
The first thing that I did was calling the clinic to schedule my medical. I was a bit annoyed that they gave me an appointment on September 9, so two days before my interview. Thus, I either had to spent two nights in Madrid or travel twice since I live about 500km away from the capital. I opted for the second option, which resulted in a busy couple of days.
The medical lasted 2.5 hours overall, including waiting time and the walk to the other hospital for the chest X-ray. I waited perhaps for half-an-hour while filling in a form, and then a doctor called me in. He asked me plenty of questions about my medical history and then proceded to do an exploration and checking blood pressure, eyesight, hearing accuracy, heart beating and so on. He was very nice and chatty.
