The representative spoke English which wasn't an issue for me as my English is quite good. I waited two hours before being interviewed. Overall it wasn't that bad for me. Just make sure you have all of your information.

We had our interview today, 12/20/15 my time, 12/21/15 her time in Tokyo. The appointment was at 9:15am and my fiancee got there around 8:45am. Once she arrived she went through the checkpoint and got a numbered ticket (#39) then gave an employee some of the papers such as her passport and DS-160 and I-134 and went and sat down. Then after waiting they called her number again and she went and got fingerprinted. She sat down and waited some more and they called her number a third time and she went into the interview room. She said it was an open room kind of like a bank, with everyone giving their interviews. She went to the man who was interviewing her and she stood while talking to him.
He asked her to basically swear-in and promise to tell the truth and that all the paperwork was up to snuff. Then he started conducting the interview and only asked her a few questions. The questions that she remembers were:

The interviewer asked:
Have you ever seen his parents?
Has he ever seen your parents?
How did you meet him?
How do you contact him?
What did you get him for his birthday?
The interviewer was very polite.

Very simple process. My fiancee was asked the name of the site where we met, and blanked on that, but she followed it up by explaining that we had spent a month in Japan together, and the interviewer was satisfied with that. The entire process took less than two hours from entering the Embassy until leaving.

It was actually the same day as my friend's interview too so the whole waiting was very easy and relax for me( thanks to my friend ).
My friend was called first and her interviewer seems so kind and nice. I could see her smiling and laughing during interview. So I was expecting to be the same but it was a lot different. My interviewer(female) are so cold (looks and sound) from beginning till end. Also she asked me some tricky question too. I got approved BUT she didn't even say congrats or anything.....only said I got approved n will get my passport later by mail.....oh well.
It all depends on who your interviewer is for real.
