Fiancee arrived only 15 minutes early and waited less than an hour.
They asked only a few questions:
* How we met
* If we are engaged
* How I proposed
* If we met each others' families
* About a vacation photo
* About a previous work visa
The interview took about 15 minutes. Interviewer was very polite and took time to explain the rest of the process.

My fiancée arrived at the embassy and waited for about an hour for them to call her name. Once called they took all our paperwork and instructed her to sit down and wait some more. After about 30 minutes she was called into her interview. She was asked about five questions all relating to how we met and when we met. Overall it was fairly easy.

My wife speaks zero English and had a very easy time doing her interview. The staff was incredibly helpful, even though we were missing some documents. They let us email the documents we needed and three days after I emailed them the passport showed up on our door.
Just be friendly and polite, and everything will be fine.

Tokyo Embassy was very efficient. They let us in early. The process was fast. Documents, finger prints, and interviewed the wife. The consular officer was polite and friendly and made jokes. The questions were light and basic. Nothing difficult. Was an easy process and we even forgot an old expired passport, but was still approved. There are reading materials and TV provided to pass the time. Total time was about an hour start to finish.
