The whole process was very easy on the day of our interview.

We arrived 20 min. before our 10 am appointment and as soon as we walked in the door, our number was already being called.
We went up to the window and gave all of our documents to the attendant who then gave us instructions on how the rest of the visit would go. Maybe 15 min. later, my husband was called back up to give his fingerprints. 10 minutes later, he was called to another window for his interview. The woman was very firm and serious and asked him the basic questions that others have reported and the following:
-What do you do here for work?
-Where do you meet?
-How did our dating life progress, etc.?
At the end, she told him he was approved and that was it. The whole process took less than 1 hour. 2 days later, we received our packet and his passport with the visa in it.

We were lucky enough to be expedited. Our experience matched what other people have said about the Tokyo embassy. Arrived on time for the appointment. Submitted all the paperwork. Waited an hour. Did fingerprints. Waited an hour. Did the interview. Maybe 5-10 minutes of questions. They've made up their mind at that point, so they're just looking one more time for any obvious flaws in your case. If you've got all your paperwork together you will have no problems.

Friendly and professional staff. No harassment. Mostly basic questions about relationship. US citizen (fiancee) had lived in Japan for four years, what job she was doing, how we met (mutual friend at a dinner party), etc. Approved on the spot. Very smooth process. Answered questions in Japanese and basic English.

Overall everything went smoothly. I had a late morning appointment though I don't think it matters whether you arrive super early or not. The time spent in the embassy will be the same.
You are allowed to use your phones indoors (and internet) so even if you have to wait, it's not an issue. The questions were very straightforward. I initially went and submitted the package when they called my number, then waited again, then gave fingerprints, waited some more and then finally had the interview. As noted, nothing difficult, just generic questions about meeting parents, where we met, what my husband (USC) is doing now etc. Didn't take longer than 5-7 minutes for the interview itself though with waiting it took me about 1,5h I think. Processing after that was super quick, I had my passport in hand in 2-3 days.
