Medical: easy but long. make sure you get all possible shots before you go (MMR and tetanus) because its super expensive to get even just one there (I had all vaccines, except for one of the MMR - which is a 3-vaccine shot.. they charged me like 50 euros extra). Other than that, the doctors are really nice.
Interview: I had 2 J-1 visas before, and one of them had the 2 year rule on it. The advisory opinion said i wasnt subject, but the consular officer wasnt satisfied because the visa said otherwise. Fortunately, I also had a no objection statement from the Italian Embassy in DC saying they didnt care whether I had the 2-year rule on it or not - when I remembered I had brought that letter, everything went smoothly and the consular officer only asked a couple of quick questions and was really really nice and understanding.

I got my medical the day before the interview. I got at the Instuto Varelli at 7.40 am and I was out by 11.30.
The doctores are nice and friendly. I got blood test, X Ray and they gave me the 2 shots I was missing and just a general control at the end. No particular questions.
Even at the Consulate everything moved pretty quickly. My appointment was at 10 am and I showed up at 9.30...By 10.20 I was out. They didn't ask for any eveidence, they already had the whole package from CSC. The Consular was very nice!! He just asked me if my fiancee was still deployed and where, when he was coming back and when and where we were planing to get married...Then he just told me "Well this time you won't have problem to get to the States cuz you will have your Fiancee Visa" (last time I tryed to enter they deny me entry) and he made me a big smile and showed me two thumbs up...Haha! He was a pretty funny guy! Anyways I got say everything went very good!!

Medical exam was held on a Monday morning. There were 5 or 6 others in waiting room. All employees were very nice. The appointment took less than 2 hours total. They would have sent me direct to the US Consulate following the exam, but it was Columbus Day (recognized by US Government, office was closed.)
Next day was US Consulate appointment. When called to the window, all documents were handed over and checked. No questions were asked. They said I could come back by 5pm that same day to see if my visa was ready. I did and I recieved it by 4:45pm the same day!
In general, the process was fluid and positive. Good luck to everyone!

I want to start by saying that, like everyone, my fiance and I were nervous about his medical exam and interview for the K1 visa (August 2008), but that the process went as smoothly and as easily as we could have hoped.
Our day began by attempting to find the Varelli Institute. Originally we had planned to take a subway and walk the rest of the way, but we found the subway system to be enormously confusing and when we attempted to take a bus it happened that both drivers were taking a break. Furthermore, the Institute is in a residential district and quite far from the city center.
Coming from the main train station terminal we found the best way to get to the Institute to be to take Metro 2 on the red line to the stop at Campli Flegrei and then to take a taxi (081.20.20.20) to the Institute. We did not arrive until nearly 8:15 but the whole process moved swiftly and my fiance was done by about 10:30. On July 11 we arrived at 7:45 am and were seated outside, due to construction, in the compound. My wife had done the medical the day before, and there were no issues. We were number 13 but after number 4 was a called a man came out and asked who had been there the day before, which we had, so we,and the other K visa folks (and some others) were allowed into the waiting area. After about an hour we were called up to the window, and not even asked any questions. my wife had to "swear to tell the truth" and then we were told to come back at 4pm that day for the visa. So we showed up, and they kept everyone outside and at 4PM the same man came out and handed out the sealed packets and passports with the visas. Overall for us much easier than we had thought. now granted it took us one calender year from filing to get there but that was about what our lawyer said it would take
