Everything went smoothly and I can't complain about the process.
The medical exam was fast and easy, not much waiting at the consulate either, the interview was easy as well. I didn't get the visa the same day because I didn't have a document (completely my fault) but I received my passport back one week later upon sending them the requested document.
I give a 5 to the process and a 1 to the venue (Napoli is chaotic, dangerous and dirty, and I couldn't wait to leave), so 3 to the whole experience.

I was extremely nervous about our visit (for my Italian husband), but everything went just fine.
We arrived for the medical examination at 8:15, and there were already several couples ahead of us. We gave my husband's passport to the receptionist, then after about 20min they called his name and he got blood drawn. Then we went downstairs and waited about an hour to be called in to give them my husband's vaccine records and a photo. He was missing a second dose of MMR, so we paid a total of 206 euros. We paid in cash but you can go back upstairs if you have to pay with a card. Then we waited again another half hour, and they took a group to go get their chest x-rays done. That was quick. But then we waited another hour or so for the medical visit with the doctor. (There were a few families and they got to go first. Counting children there were probably around 40 people there.) They were very nice and efficient. I went in March so it was not busy at all and I was in and out in 1 hour on the day of the interview! The medical took approx 2 hours-- there were 8 of us and they have the names on a list so the order is pretty much random. Overall really good experience and much better than the Milan Consulate!

I went to Naples the evening of the Valentines day because on monday i would attend the medicals and on tuesday the interview.
I've found a nice, cheap and cozy B&B close the Mergellina's station, so less than a quarter mile from the Cosnulate. They reserved me the taxi for the next day, at 7.20 am, to go to the Istituto Varelli (7 euros ride). Blood exam done, medical interview, visit, two shots (tetanus and MMR) and chest x-ray, at 10 am I was out of there and i've spent the morning to make a tour of Naples (first time for me there). The afternoon has been like a nightmare: I've controlled several times all the papers and i was in fear that something could gro wrong (I believe in Murphy), during the night i couldn't sleep well at all and as i went out the B&B surely I seemed a guy sent to the death row.

Everything went smoothly, and the man who conducted the interview was very kind. It helps A LOT if you speak English at the interview.
