    what does daniel have to do with his medical results??? does he have to do the I-693 even though he had his medical and all of his shots? I'm confused!

      I don't think so. Read the instructions of this form (http://www.uscis.gov/files/form/i-693instr.pdf). I would only submit the copy of the vaccination record I got during my medical (DS-3025). Anyone, correct me if I'm wrong.
