They do not say they check each application. they may but that would surprise me, obviously there are people who qualify for a DL in that age and gender category but do not need to register.

  11. The information they require in the vast majority of cases is on the application form. No harm taking anything else but very unlikely to be asked for. I do remember the NHS had a list of recommended vaccinations depending where you were going, however I do not think there were any for the US.
