Civil Surgeon complaint

Just got a bill in the mail from the clinic for $333 for blood tests. When I called to make the appointment nothing was mentioned about this. They said $250 for the medical and extra for any vaccines required. I even called again a week later just to double check the prices and that's all that was said, $250 for the medical exam and extra for required vaccines. When we were physically there nothing was said about this extra cost, they just gave us a paper and told us to go somewhere to get the blood tests done. Is there anything that can be done? The clinic has all calls recorded so my call to double check the prices should be easily proven. -

Just got off the phone with the clinic, they claimed the envelope was sealed correctly when it was sent out. They refused to say anything about them not sealing correctly according to the instructions. They are saying that for a fee of $50 we can send back the envelope so they can reseal it. Incredible. -

Called the Civil Surgeon clinic and they told me to e-mail them pictures of the medical envelope. I did as they asked and I am waiting to see what they see. Again, just to give context, the envelope containing the medical was not sealed properly according to the I-693 Instructions -

I can only e-mail since I am in the U.S. right now but I do not think they would fix it. It was long ago and I do not think they would react positively to me contacting them after all this time to say they made a mistake. Even if they fixed it, USCIS has the original DS-3025 so I'm not sure how that would work out. -

I got a physical copy when the results of my K-1 medical exam were ready and I picked them up. It's been a while since my Medical, I just didn't know the correct procedure back then so I didn't say anything when I got my DS-3025. Would it be an option to just send the DS-3025 and the last 2 Hep B doses in with the I-485? Technically my DS-3025 is COMPLETE, the panel physician just incorrectly marked it. So would I but unfortunately from the state of the envelope I received in the mail, I don't think they know. I looked at the Form I-693 instructions and this is what I found: The envelope I received does not say have the "Do not open" written on it. There is no clear tape covering the doctor's signature. The envelope is open in one of the ends so it has been altered. I tried to add a picture but it does not work. -

Both problems are with the same doctor. The same Civil Surgeon in the U.S. He refused to accept my DS-3025 and said the U.S. Government doesn't accept it, he filled out the I-693 incorrectly and now they sent out the sealed medical incorrectly. They do not have a better idea of what they need to do, they don't have ANY idea of what they need to do. I don't know what people keep thinking that. I pointed them towards the I-693 Instructions and the Technical Instructions for Civil Surgeons, both documents that they are required to follow. They did not care. They had no idea what a K-1 visa is and what the procedure for that is and thought I was lying about the whole thing. They did not believe me when I said that according to immigration law, I had to complete a medical in my home country and get my vaccines documented. I've been trying to clarify these concerns with the clinic for 10 days, they are simply not listening and do not care what I have to say. I want to file a complaint because it is not normal to take people's money, do things incorrectly and then refuse to do anything to fix it. We paid for a service and we did not get what we paid for. It seems like an open and shut case to me. The Civil Surgeon has to follow the official guidelines and rules, and he did not. I tried to point out these mistakes in order to get them fixed and they flat out ignored me. Just to clarify, we will most likely be going to a different Civil Surgeon so there is no problem with the AOS. I just want to file a complaint because of how horrible they handled this whole situation. -

I have not called about the envelope as we got it late on Friday and it's still the weekend. Going to call on Monday but I do not expect any kind of help. I have called the clinic a dozen times since the medical and they have refused to help or do anything at all. I only got to talk to the people at the front desk and they seem uninformed about the procedures and instructions for the I-693 form. I was told to e-mail the assistant nurse all this information since she should know more but the only thing she did was call my wife and said "Your husband sent me a very long e-mail but there is nothing we can do since the envelope has already been sealed and sent out". It seems they are just refusing to help or even talk to me anymore so I am not expecting anything at all. I am in the process of AOS-ing, my DS-3025 was not marked complete even though it technically was so now I have to go through a civil surgeon. My DS-3025 has all the required vaccines with a B Waiver for the Hep B as there was no time for the last 2 doses, however the panel physician did not mark it as complete and told me to get them. I now have the last 2 doses of the Hep B, would it be possible to just send the DS-3025 in the AOS package with the proof of the 2 doses and a paper explaining the situation? I am just trying to find the best possible solution in order to AOS and not receive an RFE to further prolong the process. -

The I-693 arrived in the mail along with the copy. The envelope for the I-693 is opened in a corner, is that normal? The I-693 instructions say USCIS will return the medical if it is altered in any way. And the doctor's initials are over the seal when the instructions say they have to be under it, is that correct? Also the Civil Surgeon wrote "No documentation for Mumps and Rubella provided" but my vaccination record from my K-1 medical is literally in the documentation at the back showing MMR. Is this not a HUGE mistake? If the Instructions say K-1 visa holders are eligible to do the vaccinations part only, are they not breaking the rules? I am not talking about the prices, I'm just simply saying that they have to follow the instructions and the instructions say K-1 holders can skip the full medical. Unfortunately there are only 6 or 7 civil surgeons in our location so the options are limited, would have to go out of state for anyone else. I don't mind paying for the full medical as long as everything is done correctly, which it was not. -

Does that exempt them from following the rules? Question mark? Snarky answer? https://www.uscis.gov/policy-manual/volume-8-part-c If the Instructions for Form I-693 say a medical exam is not necessary and they force people to pay full price, does that not mean they're refusing to comply? -

I have, everything is marked off on my DS-3025 except the Hep B which got waived for the last 2 doses. The panel physician did not mark it as complete for some reason though so I am forced to go through a civil surgeon. Are Civil Surgeons allowed to force the full medical? The I-693 Instructions mention the specific K-1 case where only the vaccine part is done and they have to follow the instructions. -
Does anyone know what the official procedure is to file a complaint towards a civil surgeon? There are limited options regarding Civil Surgeons in our state and we picked the closest one to home but have had an awful and really stressful experience. As a K-1 I only needed the vaccine history transferred and I ticked the box on my I-693 form and told them but they had no idea what that was and made me have a new medical just so they can get more money. They refused to transfer the vaccine history from my DS-3025 saying the United States Government does not accept this document. They had no idea what it was at first and then they "called" someone and came up with this answer. I have pointed them to the Department of State and the CDC Websites as well as the I-693 form Instructions Guideline that make note of the DS-3025 document but they won't budge. They also managed to screw up my I-693 and not mark off MMR but only Measles because of a misunderstanding with my family doctor vaccine record after they refused my DS-3025. I got an updated vaccine form from my family in my home country and I called them before they sent out the form and let them know but they just sent it out anyway. Now they are refusing to redo the I-693 and correct their mistake. Have called multiple times trying to explain and resolve this but they are beyond unprofessional and incompetent and I do not use these words lightly. Just to be clear, we will most likely using another Civil Surgeon and getting a new I-693 but I still want to file a complaint. Far too many people just suck it up and say that's just how it is but in my opinion this is misconduct and abuse towards patients.

I think there is an Immunization database in my state but I'm not sure how it all works. Does it cost anything? Would they accept my DS-3025? The civil surgeon and their clinic had no idea what the DS-3025 was and refused to transfer it on the I-693 falsely saying that the United States Government does not accept it. As an update to where I left this at, I contacted the Civil Surgeon explaining the misunderstanding and told them I have a corrected vaccine record showing that I do indeed have the MMR Vaccine and they said it was too late since they already sent out the I-693 form through mail. Is there anything I can do to make them rectify this mistake? I paid $250 for this and have been treated horribly and accused of lying.

Very unfortunate. I managed to contact my family doctor in my home country and I obtained a vaccination paper rectifying the mistake and showing that I do indeed have the MMR Vaccine and not just the Measles. Is it possible to get the civil surgeon to rectify HIS mistake and mark it off as completed so I don't have to do another medical?

I know that, that was a mistake on the doctor's part. I have everything needed on my DS-3025, she waived the other 2 doses of the Hep B but I got them later, she just marked it wrong for some reason instead of saying it was complete. Wouldn't USCIS just see that I have everything needed for complete the requirements?

I just checked my DS-3025 and I had a couple of questions. My Hep B vaccine on it has a B waiver marking insufficient time interval to complete the series. Wouldn't that mean I can just send the DS-3025 in instead of the i693? The doctor did not mark "Immigrant Visa or Parolee applicant completed vaccination requirements" but "K Visa applicant electing not to be vaccinated at this examination". Need to specify I was never asked anything as they don't offer vaccines there and the only thing I was told was I needed the last 2 doses of the Hep B. I have DTap, TT (Tetanus), IPV (polio), MMR, Varicella history, Influenza and Covid marked off on it. Am I missing anything else?

The civil surgeon is okay with filing and signing the I-693, he just refused to use my DS-3025 as proof and won't mark my MMR and my Covid (even though I told him the Covid shot got waived by the latest requirements). He will be adding my DS-3025 and my vaccine paperwork from my home country as extra documentation on my i693, he just would not accept the DS-3025 as proof and won't sign those 2 off.

The only vaccine papers I have are from my family doctor documenting everything I've had since I was a child. The system is very out of date compared to most Western countries and everything is on paper. These go back to the 90s so I'm not sure if they have the serial numbers/batch numbers in their official ledger but for my record they only put the dates in. They mark everything needed but the doctor's sheet has no section for MMR, only Mumps so my MMR was marked there.

I called my overseas physician to get a list of the required vaccines when I made my appointment but I could not get the last 2 doses of the Hep B since they had to be spaced out. That's not an error. I had everything else completed. They should have marked me off as completed since there was not enough time for the last 2 doses.

I showed my original vaccine records from my home country along with a translation, there was only a small issue. On my family doctor's vaccine chart (Eastern European country so not the most up to date on paperwork and stuff like that) there is no place to mark Mumps, there is only a Measles rubric. So my MMR was just marked there and the civil surgeon refused to acknowledge that I have the MMR even though it is marked on the DS-3025. I tried to explain but they did not believe me.

My DS-3025 is marked off as K Visa applicant electing not to be vaccinated at this examination. I was not asked if I wanted to get vaccinated as the place did not do vaccinations and it was the only one in my country. The doctor simply told me I needed 2 more doses of the Hep B and to bring those along with my DS-3025 to the civil surgeon. Unfortunately In my location there are only 2 Civil Surgeons, the one I went to and another one that charges an extra $300 for blood work so this was my only choice. I checked the report on my visit on the clinic online portal and they marked me off as not having the MMR and the Covid even though my DS-3025 clearly has them marked off. I feel like they screwed up from the beginning of my medical as the first thing the nurse told me when I went after they checked all the documents I provided is was that I did not have any vaccinations at all and refused to let me explain or ask anything. I did get fully vaccinated overseas. I had everything but the last 2 doses of the Hep B and the doctor told me to get them and bring them to the civil surgeon. I have every single thing marked off at the time of my medical except the 2 Hep B doses but my DS-3025 is not marked off as complete.

Hello, I am very distraught following my medical in the US for my i693. I had my medical yesterday and as a K1 visa holder I have previously had a medical in my home country and was given a DS-3025 because I was missing my last 2 doses of Hep B so I was told to get these and bring them with the DS-3025 to the civil surgeon for them to mark off my vaccines. However during my medical I was told they will not accept the DS-3025 as proof of vaccinations because it does not have lot numbers on it. I tried to explain it is an official document and I was told anyone can fake it and it is not guaranteed the US Government will accept it. I know I am in the right here, is there anything I can do? They had not even heard of the DS-3025.

I tried looking this up but I could not find anything relating to these specific circumstances. K1 finally got approved, moved to the US, married in Las Vegas the next month. Have not applied for AOS yet. Here lies the problem. For personal reasons I want to change my last name to my grandmother's last name and my wife would be taking it as well and it would be our (married) family name. I could not find any relevant information about what to do in this situation. Some people have said to just apply for AOS under the new name, some people say your chosen name has to appear somewhere on the marriage certificate. What do I do in this situation? I would really appreciate any kind of help. If possible, I do not want to AOS under my current last name.
