Hello, I have a couple of questions about letter “REQUEST FOR EVIDENCE” under, SECONDARY EVIDENCE, Church records or Religious documents, do you happen to know what kind of information that is needed? I am confused as right under Any evidence submitted should include 1. Name of the child and child's parent (s) which I understand but confused about the second and third questions. 2. Dates. I'm wondering if the dates of the childbirth, religious event, school attendance, hospital admitted, etc 3. And " Stamps, seal, signature of issuing official, authorized personnel, etc" All information must be included in the church letter. Please let me know what kind of information is required. I am in the process of obtaining the English version of my birth certificate as requested. Just to clarify, should I submit the English version along with the old one that I had previously submitted? I am a bit confused about this. In the first question, "include a translation of the registrar's name and information, signature and stamp of the civil authority". Would it be ok if the hospital wrote a new birth certificate in English? I'm not sure about "including a translation of the registrar's information". However, they would still sign and stamp it just like how the old one looks like. Thank you,
