  1. I checked with my wife and looked at past chat messages with my parents and wife and yeah they had no HepB vaccines on the day I was there. Cost wasn't the problem, I just wrongly assumed it was. I'll contact visamedicals and ask if they can correct the DS-3025 or not
  2. Yeah I can't imagine why else I'd have declined to have it while I was there haha It cost $0 at walgreens
  3. Td, IPV, Hepatitus B and COVID-19 say "K1" under the "vaccine given by panel site" column - DTP, DTaP - Td - IPV - MMR - Varicella history are ticked
  4. "K visa applicant electing to not be vaccinated at this examination" is checked - IIRC I wanted to try and get it on the NHS as visamedicals wanted to charge me for it Under the "Vaccine Given By Panel Site" column for HepB it says "K1". There's no letter for the waiver column
  5. Hi! I wasn't able to get the HepB vaccine before my VisaMedicals appointment in June last year so the DS-3025 doesn't include it. Now I've had the vaccine (thanks walgreens!) and have the receipt from that. Do I need to redo the entire medical examination using a civil surgeon now or can I just get a civil surgeon to transcribe the DS-3025 + the hepb vaccine onto the I-693? I am having trouble finding a civil surgeon in Virginia that will just transcribe it. Any suggestions? Thanks, xkl
  6. Ok yeah I guess there's no way to have it collected from me - travelling to one of a dozen DX facilities in the UK is the only option
  7. Hi I was a numpty and didn't realise the birth certificate I have is a short form certificate. The embassy have asked me to send them the long form certificate I followed the instructions in the email and generated a courier-in receipt Do I now have to travel to a facility listed on https://ais.usvisa-info.com/en-gb/niv/information/courier_dropboxes or will they collect it from my home?
  8. It went well, haven't been asked for more info so the Patient Care Summary from the GP receptionist was enough.
  9. Hi! I don't unfortunately. The diagnosis for these was via CAMHS which automatically discharges people once they turn 18 (several years ago now!). I went to the GP and asked for a Patient Care Summary which included my ADHD/autism but no mention of depression for some reason, however it included the SSRI I take so hopefully that'll be fine. I've never been sectioned or self harmed or been a danger to anyone else so hopefully won't have too much difficulty. I'll update this post once I'm back. Now all I need to do is remember to go to the passport photo booth in London Waterloo station on the day and get some passport size photos (I only have the US 2x2 photos right now and they want UK size!)
  10. Hi, I have have autism/ADHD/depression. The ADHD and depression is medicated The appointment confirmation email I received from VisaMedicals says "If you have any significant/ongoing/previous medical conditions please ensure that you bring a report from your GP/Specialist/Consultant regarding this; including, medication, prognosis, management etc. (this includes conditions such as depression, anxiety, learning difficulties and hypertension)." What exactly do I ask the GP receptionist for? They were able to print off my vaccine records without a problem, but there's no way I can get a GP appointment in this time. Is there a specific thing to ask for?
