    As I understand, once you have the green card, work authorization becomes moot.

    As I understand, it has to be done by a Civil Surgeon who can 'append' the 693.

    That was indeed the case. Called the only Civil Surgeon on Maui - $340 CASH. For fun, called Costco pharmacy - $56.
